If you need to file a claim for your pet:

  1. Download a claim form - Many veterinarians also have them available right at their office.

  2. Take your pet to any licensed veterinarian for diagnosis and treatment.

  3. Pay your veterinary bill in full, fill out sections 1 and 4 of the claim form, and have your veterinarian complete sections 2 and 3. Remember to sign your form!

  4. Attach your receipt(s) or original invoice to the claim form. Be sure to keep a copy for your own records.
