What is an OCF-2 Form?

You must fill out the OCF-2 Employer's Confirmation Form if you are claiming Income Replacement benefits.

You will then need to prove that you were employed at least 26 weeks in a row before the accident and that the accident is responsible for you not showing up at work. Usually, you will also need to send it together with proof of income.

You will need to fill out parts 1 and 4 of this application form if your insurance company asks you to. You will need to give it then to your employer (or former one) to complete the rest of the form. If you have different employers listed on your Application for Accident Benefits, each of them must fill out a separate OCF-2 form.

Extra OCF-2 forms can either be downloaded on this page for free or you can ask for some extra forms from your insurance company. Your employers can return directly the forms completed to your insurance company.

Watch out!

All accidents must be reported to the police before claiming any benefit to any insurance company. If you still don't have an insurance policy explore our comprehensive guide on the top plans in Ontario here: Best Car Insurance in Ontario.
