What is an OCF-2 Form?
You must fill out the OCF-2 Employer's Confirmation Form if you are claiming Income Replacement benefits.
You will then need to prove that you were employed at least 26 weeks in a row before the accident and that the accident is responsible for you not showing up at work. Usually, you will also need to send it together with proof of income.
You will need to fill out parts 1 and 4 of this application form if your insurance company asks you to. You will need to give it then to your employer (or former one) to complete the rest of the form. If you have different employers listed on your Application for Accident Benefits, each of them must fill out a separate OCF-2 form.
Extra OCF-2 forms can either be downloaded on this page for free or you can ask for some extra forms from your insurance company. Your employers can return directly the forms completed to your insurance company.

Where to send the OCF-2 Form?
Depending on how you've been injured in an automobile accident, you must complete and return the OCF-2 Form to different companies:

If You Own, Lease, or Have Regular Use of a Company Automobile | You may be able to obtain accident benefits from the Motor Vehicle Accident Claims Fund (see Part 11). |
If You are a Listed Driver | Send the OCF-2 Form to the insurance company that insures the vehicle, or in the case of leasing, to the insurance company that insures the car in which you were an occupant. |
Occupant of Somebody Else's Automobile | If you are an occupant of somebody else's automobile that was insured at the time of the accident, send your OCF forms to the insurance company that insures this automobile. |
Pedestrian or Bicyclist | If you are a pedestrian or a bicyclist struck by an automobile that was insured at the time of the accident, send your OCF forms to the insurance company of the automobile that struck you. |
Uninsured Automobile | If you are an occupant of an automobile that was not insured at the time of the accident, send your OCF-2 Form to the insurance company of any other automobile that was involved in the accident. |
None of the Above Apply | You may be able to obtain accidents benefits from the Motor Vehicle Accident Claims Fund (see Part 11). |
